Anatomy Of The Wrist And Carpal Tunnel Surgery

  The wrist carpus (carpus is a Latin word for wrist) consists of 8 carpal bones in 2 rows of 4. The two rows form as it were two arches forming a tunnel under the vulnerable vital structures (nerves, large blood vessels and tendons). On the palm side of the wrist, the two arcs of the carpal tunnel are connected to each other by a solid wiry membrane (transverse strap), that these structures protect, that  contributing to the tension of the arc-shaped tunnel, which is the natural shape of the wrist.. The Carpal Tunnel Syndrome And Carpal Tunnel Surgery The carpal tunnel is such an important nerve that provides a feeling of being part of the hand and includes bending movements of the fingers. Carpal tunnel syndrome is a collection of various complaints that may occur when the median nerve is pinched in the carpal tunnel due to swelling of the tendon sheaths. This is known as tenosynovitis and occurs in the tunnel. It can also occur due to thickening of the transverse gradient membrane. Repetitive movements of the hands are one cause of tenosynovitis. This increases the pressure in the carpal tunnel and causes irritation of the nerve. Irritation of a nerve causes failure in the sense motor function and pain in the limb to which the nerve is connected. In this case a portion of the hand is affected.   Symptoms Of Carpal Tunnel Syndrome   The symptoms associated with CTS may vary considerably. The symptoms often occur at night and make you wake up but can also occur in daily activities such as driving or reading the newspaper. Usually you only have to change the load in the affected hand although it can occur in both hands. Below is a summary of various complaints that may occur in varying degrees:
  1. A tingling and painful sensation or tingling in the fingers and hand with the exception of the little finger and part of the ring finger.
  2. Numbness in the palm and the fingers, except the little finger and part of the ring finger
  3. Sometimes feel as if the hand is swollen
  4. Pain radiating to the forearm, elbow and shoulder 5 Sometimes loss of strength in your hand so you can just let things fall
Diagnosis And Investigation Before The Carpal Tunnel Surgery Based on the symptoms, professional diagnosis may often be needed before carpal tunnel surgery. If symptoms occur as local pressure on the nerve at the level of the palm side of the wrist, it becomes more likely for carpal tunnel release surgery to be performed. To be sure that there is carpal tunnel syndrome, a nerve conduction study is needed. This study tests the conduction velocity of the affected nerve by means of small needles and short pulses, which is not painful when carried out. This is known as an electromyography (EMG). The severity of the irritation in the nerve can then be read by a decreased conduction velocity. By means of an electromyography, a distinction between pressure on the nerve at the carpal tunnel or at the height of the shoulder or the neck vertebrae can be made.   The Non-operative Treatment Providing a relief of the symptoms can be achieved by adaptation of certain procedures. After a pregnancy the symptoms occur spontaneously: however, feelings lost during pregnancy may be a reason for existence of numbness otherwise it becomes permanent if not treated. Sometimes a medication in pill form such as anti-inflammatory drugs or an injection of corticosteroids (adrenal hormone) in the carpal tunnel is adequate. There are occasionally good results described by treatment with a high dose of vitamin B6. If this is not possible or not the desired result, surgery may be needed especially in the case of emotional and or motor dysfunction.
